docs: add AI disclosure checkbox to PR template

Browse Source 
Add checkbox to encourage contributors to disclose AI tool usage
(ChatGPT, Claude, Gemini, etc.) in their pull requests, complementing
the new AI usage policy in CONTRIBUTING.md.
